Homeschool

To enroll your child(ren) in Homeschool, please follow these steps:

Review Program Requirements
- Carefully read the Homeschool Program Requirements to ensure you understand all expectations and guidelines.
Complete Required Forms
- Fill out the Homeschooling Affidavit for your child's grade level
  - Homeschool Affidavit - Elementary
  - Homeschool Affidavit - Secondary
- Complete Homeschool Objectives/Goals Form
  - Elementary
  - Secondary
- Gather all required supporting documents.
  - Proof of Immunization
Submit Your Materials
- Submit the completed forms and supporting documents to the Homeschool Liaison:

Curriculum Materials
- If you need curriculum resources, complete the Curriculum Materials Request Form and submit it along with your affidavit.
Prepare Portfolio and Complete End of Year Evaluation
- Maintain a Homeschool Portfolio and complete an evaluation.
